What to bring with you to your appointment?
You will need to bring a copy of your insurance card with you. If you have any medical records, tests results or x-rays that pertain to your condition, you should bring those with you so the doctor can review them.
What To expect from our office.
On your first visit to our office, you will be warmly greeted by Susie or Norma. You will be given forms to fill out that provide us with your health history, a description of the reason for your visit and basic insurance information. You can also fill out secure forms online from your home, that will be automatically synchronized with our electronic records, saving a lot of time for everyone. Click here for New Patient forms.
After completing necessary forms, you will meet with the doctor and discuss your unique situation. After your consultation, an examination will be performed and if necessary, x-rays will be taken.
The first visit will last approximately 45 minutes.
